Reported symptoms include fear of mirrors and memory loss, pointing to prototypical Moonlight Syndrome diagnosis.
Symptoms progressing at moderate rate.
Scheduled for surgical procedure next week under orders from Director Haibara.
Procedure was previously performed on two Moonlight Syndrome patients with more severe symptoms, producing no positive result.
It is hoped that performing this procedure on a patient with relatively mild symptoms will lead to development of a treatment for Moonlight Syndrome. Therefore, all involved are to take detailed notes and make appropriate reports afterwards.

月幽病と診断。
病状の進行具合は中度。
灰原院長の指示の下、
来週、外科手術を執り行う予定。
なお月幽病の外科手術は
重度の患者を対象に二度行われたが
芳しい結果を得るには至っていない。
比較的症状の軽い患者を対象にした
今回の手術は、月幽病の早期治療に
結びつくと思われる為、
各自詳細に記録を残し、後日報告の事。
なお患者およびその身内に対して
上記内容の守秘を徹底する事。
