Past

As a child staying on Rogetsu Isle, Misaki Asou established a strong friendship with Sakuya Haibara. Sakuya used dolls as a way to reinforce her own identity, and gave Misaki a doll to use in the same way. Because the doll was a receptacle for both Sakuya and Misaki's identities, she was named "Miya", combining characters from both names.
Misaki and Sakuya were eventually separated by the hospital staff, and Misaki started treating Miya as a human and her best and only friend, carrying the doll everywhere with her. Misaki would speak with Miya and sleep with Miya in her bed; everywhere Misaki went, Miya went too. When Misaki's Moonlight Syndrome progressed, Miya comforted and stabilised her, and made her feel that she and Sakuya were still together.

Fatal Frame IV
After ten years, with the Lunar Eclipse approaching, Sakuya's spirit begins to call Misaki back to the island, in the form of a human Miya. Miya guides Misaki through Rogetsu Hall and Haibara Infirmary, encouraging her to remember the "faceless woman" and the relationship they shared.

In Misaki's final chapter, the truth about Miya is revealed. Sakuya assumes the human form of Miya and shows Misaki the doll, which is still lying on the ground at the centre of the stage where Misaki dropped it ten years ago. Misaki's memories flood back, and she starts to cry, holding the Miya doll tightly. Sakuya puts her arms around Misaki, who feels as if she is about to experience Blooming.[1] Sakuya vanishes, and Misaki collapses, just as Sakuya did after the Rite of Descent.
In Chapter 9, Ruka finds Miya still lying on the ground on the underground dais where Misaki fell.
The End?
Like Misaki, Miya's fate is unknown.

In the second ending of the game, unlocked after beating Hard mode, Misaki wakes up on the ground, still holding Miya. When the ghost of Madoka Tsukimori appears, Misaki drops the doll, and Miya is last seen lying on the ground.
Misc. Info
- Miya also appears in Ruka's memory of the Rite of Descent.
- Although the doll is clearly visible when Ruka arrives in the Lunar Underworld Hall, Ruka does not acknowledge or interact with her.
