Biography
Ayako was hospitalized at Rogetsu Hall when she was 12 years old, after displaying sudden symptoms of memory impairment.[1] She was referred by the hospital Director, Shigeto Haibara, and therefore was treated with respect by the hospital staff,[2] although her personality was destructive and sadistic. Her last name was purposefully not mentioned to other patients, so as not to reveal who her blood relations were. Ayako's family name may also have been the reason why her antics were tolerated at the hospital.
She had a fascination with killing and causing harm to others,[1] and would often purposely injure the nurses and other children. She bullied her fellow patient Madoka Tsukimori viciously, beating her up and pushing her down the stairs, and even cut off the head of Madoka's pet canary with a pair of scissors.[3]
Although her Moonlight Syndrome was mild compared to others, Ayako showed the symptomatic obsession with particular objects - in her case, living things that had been destroyed or mutilated.[4] She considered people her "toys", and paradoxically treated the "toys" she liked worse than the ones she disliked, as she considered that hurting them made them prettier; Madoka was her favourite.[5] She also destroyed dolls, hanging their severed limbs from the ceiling of her room.
On the Day of Tranquility, Ayako saw a man Blooming outside. She retreated to the room with the canopied bed that was connected to her normal room, and waited there for somebody to help her.[6] She was the last known survivor of the disaster,[7] but was unable to tell anyone what had happened; she could only call out for her mother, and she died six months later.[8]

Misc. Info
- It is hinted at throughout the game that Ayako is the daughter of Yo Haibara and his sister Sakuya Haibara.
- Dr. Shigeto Haibara mentions to his son Yo that Ayako is "just like [him], always getting herself into mischief".[9]
- Ayako's room is #207 "Orchid Moon" - 蘭月 (rangetsu).
- Throughout the game, she appears as a hidden ghost in Hozuki Dolls found all around the island.
- She reappears in Haunted House Mode in Fatal Frame: Deep Crimson Butterfly.
