Private detective Choshiro Kirishima wakes up in front of Haibara Infirmary.
"Where am I? Why am I here?"
Choshiro remembers coming to the island at the request of Ruka's mother, Sayaka. Choshiro was the one who found the five missing girls on Rogetsu Isle ten years ago, when he still worked for the police.
Standing in front of the infirmary, Choshiro sees Yo Haibara, a suspect he had been chasing in connection with a multiple missing-persons case that took place on the island. Yo is the son of Shigeto Haibara, head of the island and director of Haibara Infirmary.
Choshiro is certain that Yo had something to do with the girls' disappearances, as they were found underneath Haibara Infirmary.
Yo evaded capture and the case was closed, but Choshiro would not give up. He handed in his badge and became a private detective to continue his search for Yo Haibara, who is here now at the infirmary.
Could Ruka be here too? With his instincts telling him that the disappearances, Yo and Haibara Infirmary are all related, Choshiro stands before the place where he found five frightened girls all those years ago...
In the not too distant past, Ruka's mother, Sayaka, is in a hospital bed, and Choshiro is there visiting her. Sayaka begs Choshiro to return to the island and save her daughter.
On Rogetsu Isle, outside the island's hospital, Choshiro awakens on the ground. As he stands, he sees a man enter the hospital. Choshiro says his name, "Haibara!" and follows after him.
As he enters the hospital, he recalls his conversation with Sayaka before. She gave him a special flashlight, able to use the power of the moon to exorcise ghosts. He also remembers how he thought this case was over and done with, but remembers the two girls who have died and knows that it is not over.
Choshiro makes his first steps into the hospital, and several ghosts descend upon him. He uses the flashlight to push them back, and they disappear. He discovers an elevator, and remembers he used it to go to a lower level where he found the missing girls ten years ago. He turns to the left and sees the ghost of a little girl in white with hair in front of her face, who asks if Choshiro is sick too.
Choshiro proceeds to the second floor of the hospital, and hears crying behind a door. He enters and explores the area, and follows a doctor through various rooms on the floor. After finding a key, he takes the stairs back to the main lobby area where the phone rings. He answers, and the person on the other line says that things here always stay the same, and that things keep repeating themselves. The phone goes dead afterwards.
Continuing to explore, Choshiro uses the key to open another wing of the hospital and enters a prep room and a surgery room. As he enters the operating room, a doctor and a nurse stand before a gurney with a sheet over it. As they disappear, he walks to the gurney and pulls back the sheet to find a medical record about a patient called Asagi Hizuki. When he looks up, the same girl with long hair in front of her face, the girl about whom the chart is written, appears and attacks.
The doctor appears behind him, and Choshiro follows. Entering another area, he discovers a small desk and work station, and finds a book about a ceremonial dance on the island. He reminisces about how the dance is important to the people on the island, and recalls that today is the day that the ceremony and dance is supposed to occur.
He collects and key and turns to leave the area, but is attacked by the doctor in the surgery room. While moving along, he is attacked again by a patient and Asagi.
Back in the main lobby, Choshiro sees Haibara step into the elevator and take it to the basement below. He can't follow, but remembers that there is an alternate path and searches for it. He weaves through different sections of the hospital and collects two keys with different phases of the moon on them.
He returns to the main doctor's office, and sees him glide into a cabinet on the side of the wall. He finds a box within the medical records room and uses the two moon keys to unlock it, and finds a note with a sudoku puzzle inside. Deciphering it and using the numbers to finish the code on the intercom on the doctor's desk unlocks a door behind the cabinet. Choshiro slides it aside, and continues downstairs.
The basement here is rather morbid, with stained beds in some areas and all sorts of different medical gear. This includes cage-like instruments with spikes in them, various dark jars with liquids in them, and a bed with restraints and a drill on the top end.
Choshiro moves through the area and enters the area where he found the girls ten years ago. He remembers seeing Ruka in ritual attire. He walks over to her, and tells her that her mother sent him to find her.
