This is the sickroom in which she was once hospitalized...
Summary
Ruka is controlled this chapter, and is still in the anteroom off of the museum after fighting Madoka. She picks up a note left behind by Madoka.
As she leaves the museum room, the ghost of a nurse appears before Ruka. She follows her through into the Entrance Hall, up the stairs, and to the second floor office. While she follows her, a melody plays over the intercom. Inside the office, Ruka turns the music off. As she turns to leave, the intercom buzzes.
It's from Room 207, Ayako's room. She complains that her toys have broken, and she wants someone to bring her new ones. A ghost of a nurse appears and murmurs, "Not her again", before attacking Ruka.
Ruka continues into the patient wing, and finds her way into Ayako's room. She spies her at the far end of the room, in the corner. Ruka approaches her, and Ayako jumps her and attacks. When she disappears, some medical records are left behind on the bed.
After squeezing through a small door where Ayako was, Ruka finds herself in a small room. To the left, on a rocking horse, she finds sheet music and a notebook. Ruka pulls the curtain back on the bed, and Ayako appears suddenly on the bed, but vanishes soon after.
With the sheet music in hand, Ruka returns to the first floor dining room where the piano is located. She plays four notes together, and after she finishes she remembers she used to ride the elevator when she was here as a little girl. She sets off to find it.
As Ruka approaches the elevator, a woman clad in black with a hat pushing a woman in a velvet cushion wheelchair exits the elevator. She pushes the wheelchair down the hallway and exits into the courtyard. Ruka follows, but when she catches up only the woman slumped in the wheelchair is there.
She reaches her hand out to touch the wheelchair, and the woman from before pushing the wheelchair grabs her wrist. Ruka pulls back, and they both attack. The woman in the wheelchair has a limp as she attacks and moves erratically, and both attack sporadically.
As they fall and disappear, they leave behind another key. As Ruka picks it up a grey fog enters the area, and a woman with long black hair in a floating black dress creeps up the stairs. In fear, Ruka runs from the courtyard.
She returns to the elevator, but none of the buttons work. There's no power routed to the elevator. A note inside the elevator describes how to turn the power on, located in the basement. Ruka enters the basement through a storage room, and proceeds to the electricity room where she solves a puzzle to return power to the elevator.
Ruka explores the area and finds a lens upgrade, but the door locks behind her. She proceeds forward and finds a large, locked door, covered with sutras. As she tries to leave the only exit has become mysteriously sealed, and a ghost appears behind her and attacks. After she exits the door, the grey pallor appears again and the woman with long black hair chases after Ruka.
After escaping her, Ruka continues to the elevator and rides it to the third floor. She walks to another office area where a nurse appears, but vanishes. Using a memo she picked up on the desk in the power room, Ruka unlocks the door. She also finds a map that shows both her room and Misaki's room are on this floor.
