The "faceless woman" was Sakuya.
She had given one of her own dolls to Misaki.
Getsuyuu Syndrome caused her to have split personalities and led to her eventual death.
In order to slow its advancement, Sakuya kept a doll to maintain her own identity.
The doll that Sakuya gave to Misaki became Misaki's identity as well.
This was the girl in black, Miya. Miya was an anchor to Misaki's personality.
Misaki comes to the Lunar Underworld Hall in the basement and finally meets with Sakuya, who is smiling kindly. As a result of the island's ritual, Sakuya has already "Bloomed". Everyone she saw, or touched, also "Bloomed".
Miya... no, Sakuya's vengeful ghost, holds Misaki, and she herself feels the as if she is about to experience "Blooming".
