The woman with no face turns out to be Sakuya, a patient at Haibara Infirmary who gave a doll to Misaki when she was young.
In the advanced stages of Moonlight Syndrome, patients' personalities split violently, leading to their deaths. In order to hold off development of these symptoms and retain her personality, Sakuya had started to regard her doll as being herself.
The doll given to Misaki took on the identities of both Misaki and Sakuya. They combined into one personality called Miya, the girl in black who Misaki remembered.
Arriving at the underground Temple of Tsukiyomi, Misaki finally confronts Sakuya. However, the gentle smiling face that Misaki once knew is gone.
As a result of her role as "Vessel" in the Rite of Descent, Sakuya has "blossomed."
Anyone who sees Sakuya's twisted visage also blossoms.
Misaki is grabbed by Miya — no, the wraith of Sakuya — and feels herself starting to blossom as well...
