Rather than urging Mio to complete the ritual, Mayu, possessed by the vengeful spirit of Sae Kurosawa, attacks her. Using the Camera Obscura, Mio is able to defeat Sae's ghost. As she takes the final shot, Mayu and Sae are forced apart and begin to fall backwards into the Hellish Abyss. Dropping the camera, Mio rushes to the edge of the Abyss and grabs Mayu's hand. Sae plummets with a shrill scream.
As she holds Mayu dangling from the edge of the Abyss, Mio recalls Itsuki telling her "Don't look down there". Instead of heeding his warning, she turns her head and looks past Mayu into the darkness. A look of terror crosses her face as she sees a mass of souls swarming below.
After pulling Mayu up, Mio kneels on the ground covering her eyes and shaking her head. She explains that she does not remember how she and Mayu were able to leave the Lost Village. The twins were found unconscious in the woods, and the village is now underwater.
- "I can't remember how we were able to leave the village. We were found passed out in the woods... and... that village is no more."
- ―Mio Amakura Hellish Abyss Ending
The twins are later seated on a bench overlooking the lake that now covers the village. Mayu turns to Mio and tells her that it's time to go. Mio only nods in response, and the twins rise to leave. Mio's eyes are now bandaged; she has been blinded from looking into the Abyss. Mayu smiles, content with knowing that her sister will now be dependent on her. As the twins exit, Mayu is heard whispering, "Together... forever."
