It was a likeness of Akane's twin, Azami, who was a Crimson Sacrifice. After performing the Crimson Sacrifice Ritual successfully, Akane became overcome with guilt and sadness at the loss of her sister. Her father decided to make a life-size doll of Azami to comfort his daughter.
The doll was so life-like that Akane treated it as though it were really her sister, and spent all her time talking to it. Because of this, it became possessed by an evil spirit and began to control her.
When Yoshitatsu discovered the doll walking by itself, he realized it was possessed and began searching for a way to destroy it.
